Medical Cards and Doctor Visit Cards at Carrick Medical Centre

You may apply for a medical card or doctor visit card under the GMS (General Medical Services) scheme. You will need to document details about your and your spose's income, your medical expenses, and other financial details such as social welfare payments and savings. 

To apply for a medical card, download the medical card application form here and bring the completed form to reception at Carrick Medical Centre.
If you are not entitled to a medical card, you will automatically be assessed for a Doctor Visit Card.
